Is this the Dixie Cup form you fill out to attend the event. Also where do you find the form for the beer you are entering?

I would recommend doing the online registration form.

You can enter your beers electronically, it costs less than paper, and you can pay for the entire event and beers via paypal.

I'm taking it we do not have a Dixie Cup registration club account?

You need to register as a brewer, then pick Dead Yeast Society from the dropdown box. Dixie Cup knows about the DYS.
